Forms from embassy

Immigration Links - Immigration Forum: Immigrant Visas - Family: Forms from embassy

I received a pile of forms from the us embassy today for the fiance visa. I have already started the K1 application and it has been filed with the INS in america. I received "Additional instructions for applying for a non immigrant visa to marry and reside in the united states" set of papers to include a ds230, of156, ds156k and i-134 forms. My question is regarding the I-134...the affidavit of support form. Do I need to get my us citizen fiance to fill this in since he will be supporting myself and my daughter when we arrive in the united states? I ask because we already sent one of those off to the INS with the first part of the visa application (129 and g325 forms). Do the embassy in london need another affidavit of support? Also, does my fiance need to fill in one form for me and one for my daughter even though she will be accompanying me? I just need to know if i have to send him the affidavit to fill in and return to me so that i can send it to the embassy along with the nonimmigraant application or if the INS send the one he has already done along with the approved petition to the embassy.

I have another question...
When I send the completed forms to the embassy in london, do I need to send photocopies of documents with them (ie, birth certificates, divorce decree etc etc). I am unsure because on one form it says to attached certificates etc and on another it says to only send the forms and not the documents. This is soooo confusing! Do I just send the completed application forms for the fiance visa for myself and my daughter or do i also include copies of documentation? And what injections do I need to get done, if any, before the medical at the embassy?

Dear Julia:

Yes, your fiance needs to send in the I-134. He should send one copy for you and one for the child. You should attach a copy of the requested documents and state that you will produce the originals at the time of your final interview.

As for the medical, contact one of the listed doctors and he or she will provide you with the information you need about any vaccinations required. Good luck.
