LCA Form to Use if Filling Out by Hand or Typewriter

AILA has provided the attached LCA form to use if you are filling it out by hand or typewriter. Please be sure to fill in a page link number on the bottom left corner (any number, other than 313775, will do), and that the number is identical on all 3 pages. Be patient--the form is slow to print.

This form is strictly for those who wish to print out a hard copy and complete the form by hand or typewriter. It is better to use this form for this purpose than the form at the DOL website. The DOL uses the page link number on the bottom of the form to keep the three pages of a single filing together. Since the form on the DOL website uses the default page link number of 313775 if you print out a blank form, the DOL has already received more than 1,500 filings with the 313775 page link number and is unable to determine which pages go together to comprise a complete filing.
On the version of the form that is attached, the page link number is not completed. You will need to complete it, with any number you choose (other than 313775, of course). Just be sure that you use the same number on all three pages.
