Subject: A NEW FORM - DS-3052, NONIMMIGRANT V VISA APPLICATION

1. The Department announces the introduction of a new
form, Nonimmigrant V Visa Application (DS-3052).
Effective immediately, V visa applicants will use this
form as a supplement to the OF-156, Nonimmigrant Visa
Application.
2. The DS-3052 should be available on the A/RPS/DIR
Intranet site (http://99.1.1.27/), by Monday, March 12.
This site is the official repository for all Department
forms. Posts should be able to access that site through
their Intranet connection. It will soon be available to
the general public on the Department's Internet site
(www.state.gov).
3. The General Services Administration will not stock the
DS-3052. Posts are therefore responsible for printing
this form locally. Posts are reminded that the DS-3052 is
subject to the Paperwork Reduction Act of 1995 (PRA),
which is administered by the Office of Management and
Budget (OMB). Pursuant to the PRA, OMB gives approval for
agencies to use "information collections" like the DS-3052
for a maximum of three years at a time. Posts may not/not
make any changes to the text of the DS-3052, nor may they
modify the layout or typeface. Posts may, however,
translate the form, provided that the translation is
accurate and that the layout of the translated DS-3052
looks as much like that of the English version as
possible. No Department approval is required for
translations.
4. Because of the need to be able to issue V visas
quickly, the Department has secured emergency OMB approval
for the DS-3052. This emergency approval will expire on
August 31, 2001. Accordingly, posts should not print more
forms than they anticipate needing by this date. The Visa
Office has begun the process of securing regular OMB
approval to extend the validity of this form for three
years beyond the emergency approval expiration. We will
notify posts by septel when we receive regular OMB
approval.
5. Posts wil1 notice that the third line of the DS-3052 is
quote Use with Form DS-156 end quote. After OMB approval
of the current OF-156, Nonimmigrant Visa Application,
expires on June 30, 2001, the Department plans to
introduce a completely re-designed Nonimmigrant Visa
AppliCation. This new NIV application will be called the
DS-156. In order to avoid having to amend the DS-3052 at
that time, we decided to reference the DS-156 now. Until
the DS-156 is introduced, however, applicants will use the
OF-156 with the new DS-3052.
